Medical

Aetna Open Access POS II Plan

* The Aetna POS II Plan will be offered to all employees.

Under this plan you have the flexibility of selecting any provider for your medical services.  Eligible expenses will be considered for payment according to the Usual, Reasonable and Customary (URC) fee schedule.  Participating providers will accept the URC payment in full and take care of any necessary claim related paperwork.  If you choose a non-participating provider, you will be responsible for the co-insurance plus the difference between the actual charge for the service and the URC amount.  You may have to pay the provider and submit a claim form for reimbursement.

Local HMO Plans

* The local HMO plans will only be offered in certain areas.  Eligibility is based on home zip code. 

The local HMO plans have a number of cost-saving features designed to minimize the expenses associated with obtaining quality medical care. As with most HMO plans, all medical treatment begins with the primary care physician that you select when you initially enroll in the plan.  This primary care physician will coordinate all of your healthcare in conjunction with other specialists and facilities.  This is an in-network only plan.

Dental

The Delta Dental plan covers one (1) visit every six (6) months for preventive care services at 100% with no deductible, based on Usual, Reasonable and Customary (URC) charges. Other services are subject to a $25 individual annual deductible, or a $75 family annual deductible. Basic restorative services are paid at 80%. TMJ services are paid at 80%. Major restorative services are paid at 50%. Orthodontia is covered at 50% with a lifetime maximum of $2,000. The maximum benefit per calendar year is $2,000 per individual.

A dental network is offered. Employees are not restricted to network dentists, but there is an incentive. By using a participating dentist, the employee will not be charged any fees in excess of the negotiated rate for covered expenses (the standard deductible and co-pays still apply).

Vision

The Spectera Vision plan covers one (1) comprehensive vision exam every twelve (12) months.  The plan also covers a pair of lenses for eyeglasses or contact lenses in lieu of eyeglasses every twelve (12) months.  Frames are covered once every twenty-four (24) months.  Participants also receive access to discounted refractive eye surgery from numerous provider locations throughout the United States. Out-of-network care is also allowed by the plan.  Participants who choose to see an out-of-network optometrist/ophthalmologist will need to pay for the services up front and then submit a claim to Spectera for reimbursement.

401(k) Savings Plan

Employees are eligible to participate immediately upon employment and are 100% vested after one (1) year of service. Employees may make pre-tax contributions of 1 to 25% of their gross pay. The Company will match the first 4% of these pre-tax contributions at 100%. Additionally, employees may make after-tax contributions of 1 to 6%. Pre and post-tax contributions are subject to IRS maximum annual limitations.  Participants determine how they would like their contributions, as well as the Company’s contributions, invested in the plan.  A wide selection of investment options is available.  Participants are permitted to borrow a portion of their vested savings plan contributions under the Plan’s loan program.

Flexible Compensation Plan

The flexible compensation plan is designed to permit employees to pay for certain health care and work-related dependent care expenses with contributions made to flexible spending accounts. Contributions are made on a pre-tax basis and, therefore, are exempt from federal income, FICA and in some cases, state income taxes.

For the medical reimbursement account, the maximum dollar amount an employee can elect to contribute is $5,000 per plan year. The maximum contribution an employee can make to the dependent care account is $5,000 per plan year, per family.

Sick-Time/Short-Term Disability

Every employee receives 10 days of sick time per calendar year. 

Short-Term Disability is administered through Aetna.  Once approved an employee is eligible for 56 calendar days paid at 100% and the next 34 calendar days are paid at 75% of his or her salary. 

Depending on your length of service with the Company, you may be eligible for consideration under the provisions of the Family and Medical Leave Act (FMLA) as well.

Long-Term Disability

The long-term disability plan provides income in the event of partial or total disability. The benefit begins after ninety (90) days of continuous total disability. The employee receives 60% of his/her salary up to $10,000 monthly. Salary includes base salary, sales commissions and incentive bonuses. The cost of this plan is paid entirely by the employee, making all benefits paid non-taxable for Federal income tax purposes. Participation is this plan is mandatory.

Life Insurance

Employee Life Insurance

SunGard Availability Services provides basic life insurance to all eligible employees in the amount of two (2) times base salary, rounded to the next $1,000, up to a maximum of $1,000,000.

Accidental Death & Dismemberment (AD&D)

SunGard Availability Services provides an accidental death & dismemberment policy in the amount of two (2) times base salary, rounded to the next $1,000, up to a maximum of $1,000,000.

Business Travel Life Insurance

SunGard Availability Services provides a business travel life insurance benefit equal to three (3) times base salary, rounded to the next $1,000, up to a maximum of $500,000, for loss of life as a result of accidental death while traveling on Company business.

Supplemental Life Insurance

Employees can enroll in supplemental life insurance with a choice of 1, 2, 3, 4 or 5 times their base salary, rounded to the next $1,000, up to a maximum of $1,000,000.  This is a voluntary, 100% employee-paid benefit.  

Dependent Life Insurance

Employees can enroll in a spouse life insurance policy of up to $100,000 in $10,000 increments and/or a dependent child policy of $5,000 or $10,000 per child. There is no limit to the number of eligible children that can be covered by this benefit.  These are voluntary, 100% employee-paid benefits.

Employee Assistance Program (EAP)

The EAP offers counseling services that are designed to assist people in dealing with problems related to marital or family issues, emotional dilemmas, substance abuse, job-related problems and other conflicts. Employees and covered dependents are eligible to receive six (6) visits for all behavioral issues at no cost.  Additional counseling sessions are available. There is a toll-free hotline that is available 24 hours a day, seven days a week.

Educational Assistance Plan

This plan is designed to provide financial assistance to employees who are pursuing work-related educational courses or degree programs on an after work hours basis. The Company will reimburse employees who successfully complete pre-approved, work-related courses and degree programs, both undergraduate and graduate. Upon successful completion of the course (a grade of C or better), the Company will reimburse an employee 75% of the costs for tuition, books and applicable fees. The annual maximum to be reimbursed to an employee will not exceed the IRS regulations on educational reimbursement.

Adoption Assistance Program

To provide assistance to eligible employees who are building families, SunGard Availability Services has developed a reimbursement policy for eligible adoption expenses.  All qualified adoption-related expenses will be reimbursed to a maximum of $3,000 per child.  Qualified adoption expenses are defined to include the reasonable and necessary adoption fees, court costs, attorney’s fees, traveling expenses (including amounts expended for meals and lodging) while away from home, and other expenses that are directly related to, and the principal purpose of which is for, the legal adoption of an eligible child by the taxpayer. 

New Baby Gift Program

As a special gift, SunGard Availability Services will purchase a $500 U.S. EE Savings Bond in the name of a baby born or adopted by an employee during the course of their employment with the Company.  The savings bond will be mailed to the employee’s home directly from the Federal Reserve Bank.   

Vacation

For the purposes of earning vacation, the vacation year begins effective January 1 of each year. The number of weeks an employee is entitled to during that year is based on the years of service accumulated in that particular calendar year.  Employees are eligible to carryover up to five (5) unused, earned vacation days in to the next calendar year.   

Holidays

The Company observes the following holidays: New Year's Day, President’s Day, Good Friday, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, day after Thanksgiving and Christmas Day. Additionally, one floating holiday may be taken any time during the calendar year, regardless of an employee’s length of service.  Employees cannot carryover an unused floating holiday in to the next calendar year.
